• Reactive lesion caused by local irritation or trauma
  • Name is double misnomer – not pus producing; not a granulomatous
  • Lobulated mass that is usually pedunculated, but can also be sessile
  • Soft, fleshy, easily bleeds because of its high vascularity
  • Color ranges from pink to red to purple depending on age of the lesion; young lesions are highly vascular in appearance; older lesions tend to become more collagenized and pink
  • Predilection for the gingiva (75% of all cases); lips, tongue, buccal mucosa are the next most common sites
  • More frequently found in females due to role of progesterone/estrogen
  • Resembles metastatic malignancy – SCC, fibrosarcoma, leukemia, non-Hodgkin lymphoma, and mets foci from breast, lung kidney, prostate
  • Tx: excision with 2mm margin
